We handle all your personal data in line with the applicable legislation of the Czech Republic and directly applicable regulations of the European Union, primarily Regulation number 101\/2000 Coll. on the protection of natural personal data as amended by Act No. 480\/2004 Coll. and in line with regulation article 13 of the European Parliament and of the Council (EU) 2016\/679 signed on 27th of April 2016, General Data Protection Regulation on certain Information Society Services and on changes of some laws in accordance with later regulations.<\/p>\n

In the following text we will introduce you to the form in which we process your personal data, for what purposes we process your personal data and who are the administrators of personal data provided by you. This information is important and as such we hope that you find the time necessary to read it thoroughly.<\/p>\n

Who is the administrator of your personal data?<\/h2>\n

You give consent to the company Refrak\u010dn\u00ed Centrum Praha, ID number: 26684551, based at Olivova 948\/6, 110 00, Prague 1, registered in the Public Register kept by the Regional Court in Prague, Section C, insert 87171, in accordance with the Personal Data Protection Act and GDPR, to process the personal data as administrator under the conditions mentioned below.<\/p>\n

As personal data administrators, what type of data do we process?<\/h2>\n

Processing personal data \u2013 name, surname, date of birth, phone number and e-mail address is enabled by article 6 paragraph 1 letter b) of the Regulation \u2013 the data processing is necessary to enable us to take measures agreed prior to signing of a contract on request of the subjects of the data. It is important to process this data in order to enable communication between yourself and the administrator.<\/div>\n

Personal data on your dioptre error and your medical problems falls under a special category of personal data. This processing is necessary to enable us to recommend suitable services provided by the administrator, but it is entirely voluntary. For the processing of such information, your expressed AGREEMENT is required, and that you grant us by clicking the \u201cI agree with personal data processing\u201d button. This type of processing is enabled by article 9 paragraph 2 letter a) of the Regulation.<\/div>\n<\/div>\n

This data will be processed by the administrator for the duration of the proceedings of, however no longer than 3 years from last communication the contract should not be signed. In such a case, the communication shall remain archived for the duration of 10 years from the realisation of the last part of the service for the purpose of potential protection of rights of the administrator and also due to the valid medical services legislation.<\/div>\n

The agreement with the processing of this special category of personal data \u2013 sensitive data may be reversed at any time. Reversal of such an agreement shall result in the deletion of personal data from the company data base.<\/div>\n<\/div>\n
