This month marks a significant milestone \u2013 <\/span>30 years since the first laser eye surgery<\/b> using the LASIK method, performed by Chief Surgeon Dr. Petr Nov\u00e1k. <\/b>The procedure<\/span> not only represented a breakthrough moment in Dr. Nov\u00e1k\u2019s career but also a defining<\/span> chapter in the history of Czech refractive surgery. When first introduced in 1995, LASIK was<\/span> a revolutionary technique that opened the door to modern, precise, and safe correction of<\/span> refractive errors.<\/span>\u00a0<\/span><\/p>\n

To commemorate this anniversary, Dr. Nov\u00e1k shares his memories of the very first LASIK<\/span> procedure, reflects on the early days of laser technology, and discusses how ophthalmic<\/span> surgery has evolved over the past three decades.<\/span><\/p>\n


\nWhat were the circumstances surrounding your first LASIK surgery in 1995?<\/b>
\n<\/b>\u201cIn 1995, excimer laser surgeries were still uncommon. Just one private clinic in Prague had<\/span><\/i> been offering PRK procedures since 1993. The LASIK method was performed at only a few<\/span><\/i> centres across Europe and had not been approved at all in the United States.<\/span><\/i>\u00a0<\/span><\/i><\/p>\n

In August 1995, several colleagues and I completed specialized training at Technolas in<\/span><\/i> Munich. Consequently, in September, the Technolas 116 system was installed at the<\/span><\/i> Ophthalmology Department of the University Hospital Kr\u00e1lovsk\u00e9 Vinohrady, where a new<\/span><\/i> refractive laser centre was established under the name ELAS.<\/span><\/i>\u00a0<\/span><\/i><\/p>\n

Initially, we performed procedures using the then-common PRK method. The real<\/span><\/i> breakthrough came in October \u2014 with our first LASIK surgery.\u201d<\/span><\/i><\/p>\n


\n<\/i>Do you remember how you felt that day? Was it nerves or excitement?
\n<\/b>\u201cI was very honoured to be entrusted with the first surgery. There was nervousness all<\/span><\/i> around, but I knew I was taking part in something truly remarkable.\u201d<\/span><\/i><\/i><\/p>\n


\n<\/i>Do you recall the patient\u2019s and the team\u2019s reactions following the first surgery?<\/b>
\n<\/b>\u201cI remember that the patient only learned afterwards that he had been the very first person to<\/span><\/i> undergo the LASIK procedure. That decision was made intentionally, we didn\u2019t want to cause<\/span><\/i> unnecessary anxiety at such an important moment.\u201d<\/span><\/i><\/i><\/p>\n


\n<\/i>What made the laser technology so groundbreaking at that time?
\n<\/b>\u201cWith PRK, the healing and recovery process took about three months. We operated on one<\/span><\/i> eye first and only months later on the second eye. LASIK, on the other hand, allowed for<\/span><\/i> both eyes to be operated on at the same time, with basic healing completed by the following <\/span><\/i>day. Patients could return to their everyday activities almost immediately. It was truly<\/span><\/i> revolutionary.\u201d<\/span><\/i><\/i><\/p>\n


\n<\/i>What\u2019s your take on the fact that thirty years have passed since your first LASIK<\/b> surgery?<\/b>
\n<\/b>\u201cAt the very least, it means that I\u2019ve been around for a while,\u201d he laughs. \u201cBut seriously, the<\/span><\/i> fact that LASIK remains in use three decades later, with only minor improvements, speaks<\/span><\/i> volumes about the brilliance of its design. It\u2019s simply a genius concept.\u201d<\/span><\/i><\/i><\/p>\n


\n<\/i>What is one of the most memorable moments over the years?<\/b>
\n<\/b>\u201cI experience it every week. I always insist on personally examining patients the day after<\/span><\/i> surgery. Hearing their words \u2014 \u2018From the moment I opened my eyes, I could see\u2026 hair on<\/span><\/i> the floor, signposts I never noticed before\u2019 \u2014 it never gets old.\u201d<\/span><\/i><\/i><\/p>\n


\n<\/i>How does it feel to witness a patient seeing clearly for the very first time after<\/b> surgery?<\/b>
\n<\/b>\u201cIt\u2019s wonderful. Most patients perceive the change almost instantly, right there on the<\/span><\/i> operating table. Watching their nervousness transform into excitement and relief is incredibly<\/span><\/i> rewarding.\u201d<\/span><\/i><\/i><\/p>\n


\n<\/i>How have technologies and procedures evolved over time, from the first LASIK<\/b> surgeries to today\u2019s iDESIGN iLASIK, for example?<\/b>
\n<\/b>\u201cThe core principle remains the same, reshaping the cornea with an excimer laser beneath a<\/span><\/i> flap (lamella).<\/span><\/i>\u00a0<\/span><\/i><\/p>\n

When I started, the lamella was created mechanically using a microkeratome with an<\/span><\/i> oscillating blade. Today, we use a femtosecond laser, which is much safer and more precise.<\/span><\/i> The laser itself has also evolved \u2014 it now features a variable beam width, allowing for highly<\/span><\/i> accurate modelling of even complex corneal shapes.<\/span><\/i>\u00a0<\/span><\/i><\/p>\n

The iDESIGN iLASIK represents the most advanced approach: it begins with a detailed<\/span><\/i> analysis of all optical imperfections in the eye, from which the optimal corneal shape is<\/span><\/i> designed and then precisely created with the excimer laser.\u201d<\/span><\/i><\/i><\/p>\n


\n<\/i>How do you think public perception of laser eye surgery has changed over the past<\/b> thirty years?<\/b>
\n<\/b>\u201cFor most people, laser eye surgery has become a completely routine way of achieving life<\/span><\/i> without glasses. Over the past three decades, thousands have acquired clear vision thanks<\/span><\/i> to this technology \u2014 they are its best advertisement.<\/span><\/i><\/p>\n

It doesn\u2019t cease to surprise me, however, that some of my colleagues remain sceptical, even<\/span><\/i> suspicious, of refractive surgery. I find this simply incomprehensible.\u201d<\/span><\/i><\/i><\/p>\n


\n<\/i>What would you describe as the greatest advancement in ophthalmic surgery over the<\/b> past thirty years?<\/b>
\n<\/b>\u201cI personally find the continuous development of surgical methods and the vast expansion of<\/span><\/i> treatment options as the greatest step forward. Today we can correct almost any refractive<\/span><\/i> error more precisely and gently than ever before.\u201d<\/span><\/i><\/i><\/p>\n


\n<\/i>How do you think eye surgery will evolve over the next 10\u201320 years?<\/b>
\n<\/b>\u201cOver the next 10\u201320 years, I expect, on the one hand, further refinement of existing<\/i> techniques, but on the other hand, also the arrival of entirely new technologies. What kind?<\/i> Let\u2019s wait and see.\u201d<\/i><\/p>\n


\n<\/i>What would you like to see facilitated in the future that isn\u2019t possible just yet?<\/b>
\n<\/b>\u201cModern medicine should focus primarily on prevention. There is ongoing research on how<\/span><\/i> to prevent the development of refractive errors in childhood and that would truly be<\/span><\/i> amazing.\u201d<\/span><\/i><\/i><\/p>\n


\n<\/i>After so many years, what do you continue to enjoy and what motivates you?<\/b>
\n<\/b>\u201cFrom the moment I started studying medicine, my dream was to become a surgeon.<\/span><\/i> Surgery has its magic, you see the results of your work almost instantly. At the same time,<\/span><\/i> you carry enormous responsibility for human lives, which gives this profession a deeper<\/span><\/i> meaning.\u201d<\/span><\/i><\/i><\/p>\n


\n<\/i>What advice would you give to young doctors embarking on a journey towards<\/b> becoming eye surgeons?<\/b>
\n<\/b>\u201cAlongside skill, cultivate patience and humility. Only then will you experience that<\/span><\/i> extraordinary feeling when someone sees clearly once again as a result of your work.\u201d<\/span><\/i><\/p>\n<\/div>\n<\/div><\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n
